[ARCHIVED] Mayor Charles C. Kokoros Announces Phase 2 Vaccination Appointment Registry

Massachusetts is currently in Phase 1 of Governor Baker’s COVID Vaccine distribution timeline. 

In anticipation of phase 2, the Town of Braintree has proactively begun gathering information from residents that are 65 years of age or older and adults with one or more comorbidities interested in receiving the COVID vaccine and will be scheduling vaccination clinics when phase 2 begins.

If you are a Braintree resident and fall into one of the Phase 2 categories above and would like to be added to the vaccination appointment registry, please fill out this form: Vaccination Appointment Registry Form .

If you are unable to fill out the electronic form and would like to be added to the appointment registry, please send your name, date of birth, home address, phone number and email address to covidvaccine@braintreema.gov. You may also call the COVID vaccine hotline at 781-794-8099 and will be prompted to leave the same information on the voice mail. 

Once the Town is authorized to begin Phase 2 vaccination distribution, the Town of Braintree Health Department will contact you to schedule an appointment. 

We kindly ask that you only utilize one form of communication to share your information with the Health Department. This will help us better serve you. Additionally, we ask that you continue to contact your Primary Care Doctor or pharmacy after you have been placed on the list for additional vaccination opportunities. If the opportunity arises to get vaccinated with your Primary Care Doctor or pharmacy, please take advantage of the opportunity, and get vaccinated.
